Requesting a GP Appointment
Please use our online request form to book a GP appointment.
If you submit your request using the wrong form, you will be asked to resubmit using the correct one. A member of our GP team will contact you if this is needed.
The GP appointment request form is available Monday to Friday, 8:00am – 6:00pm. It is not available outside these hours or on bank holidays.
Please submit one form per problem. If you have more than one issue to discuss, please complete a separate form for each. If multiple problems are listed on one form, a member of the GP team will contact you to confirm which problem you would like to prioritise.

Home Visit Requests and Urgent Appointments
If you require a home visit, please submit your request before 11am whenever possible. We understand this may not always be feasible. Please complete the online request form and telephone the surgery to let our team know you or a family member needs a home visit.
For urgent medical or urgent administrative requests after 11am, please complete an online request and telephone the surgery to inform us. Your request will be prioritised and directed to the appropriate team.

Your appointment
When you complete an online request form, your query is directed to the most appropriate team to handle it. Following your submission, we may:
- Request further information by sending you a questionnaire to complete
- Offer advice via text or email
- Recommend that you visit your local pharmacy
- Offer you a consultation by phone, face-to-face, text or email. The method will be decided at the GP’s discretion.
All medical requests are reviewed by our clinical team and prioritised based on urgency – to be managed the same day, within a few days, or within one to two weeks.
If your request is not urgent, but a GP appointment is needed, you may receive:
- A message with a link to book a timed telephone appointment at a convenient time, or
- A call from a member of the GP team to arrange a face-to-face appointment
If your request requires same-day attention, a GP will contact you during surgery opening hours. Please note that we cannot provide a specific time for this call. Any appointment times displayed in the NHS App are for administrative purposes only and do not indicate when the GP will contact you.
If the clinician feels you need a same-day face-to-face appointment, a member of the team will call you to arrange this.
Cancelling or changing an appointment
To cancel your appointment:
- Use your NHS account (through the NHS website or NHS App)
- Use the GP online system: AccuRx

If you need help with your appointment
Please tell us:
- If there’s a specific doctor, nurse or other health professional you prefer to consult with
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- If you need an interpreter
- If you have any other access or communication needs
If you need help when we are closed
When the surgery is closed, the online form will not be available for GP appointment requests; however, you can still submit a routine appointment or administrative care requests.
If you need medical help or advice outside our normal opening hours (Monday to Friday, 8am – 6:00pm, excluding bank holidays), please use NHS 111 Online or call 111.
NHS 111 Online is available 24 hours a day, 365 days a year, for people aged five and over. It helps you access local NHS services quickly and safely. If you need help for a child under five, please call 111.
Call 999 in a medical or mental health emergency – for example, when someone is seriously ill or injured and their life is at risk.
